SMTD Injury Reporting Policy

The School of Music, Theatre & Dance is committed to supporting the health, safety, and academic success of its students. If you experience an injury, please follow the steps on the Injury Reporting Policy to ensure you receive appropriate care, documentation, and support for your recovery and return to class. The Injury Report form must be submitted for acute and chronic injuries as well as suspected concussions. This form may be submitted by the student, or anyone else.
