Request for Approval of a Short-Term Travel Leave of Absence

Instructions

  1. Use a new form for each request for absence approval. THE FORM MUST BE FILLED OUT IN FULL. (All fields are required except the “remuneration” question.)
  2. The maximum number of days a faculty member is permitted to be absent for professional activities or research during one term is 15 weekdays. These 15 days may be taken consecutively, or throughout the term. A term is defined as the first day of registration to the last day of the exam period.
      • For leaves totaling 15 days or less in the term, please request permission 15 days in advance of your leave start date.
      • For leaves totaling more than 15 days in a term, please request permission 30 days in advance of your leave start date. Leave requests that are greater than 15 days per term (in totality) will require both the Department Chair and the Dean’s approval.
      • All leaves should be approved prior to making commitments. Leaves that total more than 15 days away during the term may require an adjustment to your salary for the term.

  3. Indicate what provisions are being made to cover the classes or lessons you will miss. In studio instruction, it is expected that each student will receive at least 12 lessons during the term. Any external coverage needs will require a Guest Artist Contract through Jason Corey’s office. This contract will ensure a background check is completed which is mandatory for anyone teaching. Background checks require a longer lead time to complete.
  4. When you submit this form, the information will be emailed to the chair of the indicated department. After the Chair acts on your request, the Dean will be notified. You will receive an email message when all approvals are complete.
  5. IMPORTANT: You may also submit this form for approval of absence when the School of Music, Theatre & Dance is not in session or when you are not teaching. If your travel is approved in advance by the Department Chair and Dean (if applicable), and if you register your travel with the University Travel Registry, you will be protected automatically under the worker’s compensation and travel accident programs.

Register International Travel with the University Travel Registry

All faculty, staff, and students planning international travel for university-related purposes must register in the University Travel Registry before the expected departure date. The Travel Registry is a confidential and secure site within Wolverine Access for you to record travel plans and contact information. The Travel Registry supports emergency communications and provides direct access to university-approved travel abroad health insurance. The Registry can be used for domestic travel and the university recommends registering personal or leisure travel when planning a trip abroad.

U-M International Travel Policy

The university’s International Travel Policy applies to all faculty, staff, and students on all campuses. It specifies the requirements for university-related international travel, which include travel registration, travel abroad health insurance from the university’s authorized vendor, and procedures for travel to university travel warning or travel restriction destinations. Schools, colleges, and business units may have additional policies and procedures that support this overall policy.

Business Travel Accident Insurance

All active faculty and staff members traveling domestically and internationally while on official university business (excluding everyday travel to and from work) are provided coverage through MetLife for accidental death or permanent total disability. The university pays the full cost and you do not have to enroll in this coverage. See Travel Accident Insurance for more information on this benefit.

Plan Summary

  • Business Travel Accident Insurance is administered by MetLife.
  • Coverage begins on your first day of work and continues until your last day of work at the University of Michigan.
  • If you die while traveling on official university business, the plan provides a minimum benefit of $50,000 or 10 times your annual salary, whichever is more, with a ceiling of $500,000. The plan also pays benefits for dismemberment and disability.
  • The maximum benefit for any one covered accident is $10 million.

Travel Assistance

Along with Travel Accident Insurance, when traveling more than 100 miles from home, faculty and staff have access to Travel Assistance, a travel service administered by AXA Assistance USA, Inc. (AXA). Travel Assistance offers you and your dependents worldwide medical, travel, and concierge assistance services, 24 hours a day, 365 days of the year, while traveling internationally or domestically.
